Position Summary
The Hospice Liaison is the face and voice of Favoured Hospice within the community. This role focuses on building meaningful relationships with referral partners, educating families and providers about hospice services, and ensuring that every patient’s transition to hospice is handled with compassion, clarity, and respect.
The ideal candidate is a natural connector — someone who leads with empathy, communicates with confidence, and represents Favoured Hospice with professionalism and heart.
Key Responsibilities
- Build and nurture strong relationships with hospitals, skilled nursing facilities, assisted living communities, clinics, and other referral partners.
- Provide compassionate education to patients, families, and healthcare professionals about hospice philosophy, eligibility, and the unique support Favoured Hospice provides.
- Respond promptly to referral inquiries and collaborate with the intake and clinical teams to ensure timely assessments and admissions.
- Participate in community outreach efforts to increase awareness of Favoured Hospice’s mission and services.
- Maintain accurate documentation of outreach activities, referral trends, and community needs.
- Represent Favoured Hospice at community events, facility meetings, and professional gatherings with warmth, integrity, and professionalism.
- Uphold Favoured Hospice’s values of compassion, transparency, and family-centered care in every interaction.

Requirements
Qualifications- Experience in hospice, home health, senior care, or healthcare marketing preferred.
- Strong understanding of hospice philosophy and eligibility (training provided).
- Exceptional communication, presentation, and relationship-building skills.
- Ability to work independently while staying connected to a supportive team.
- Reliable transportation and valid driver’s license required.
- A compassionate, patient-centered mindset and a genuine desire to serve families during vulnerable moments.
