ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:

  • Conduct initial and ongoing comprehensive assessments of patients and families
  • Perform physical evaluations and document medical history
  • Develop, initiate, and revise individualized plans of care
  • Use health assessment data to determine nursing diagnoses
  • Include patients and families in the care planning process
  • Administer medications as prescribed
  • Provide health education and counseling to patients and families
  • Assist with daily living activities and promote comfort care
  • Serve as RN Case Manager for assigned caseload when designated by Clinical Supervisor
  • Coordinate care and collaborate with team members to ensure continuity

Clinical Documentation & Communication

  • Accurately complete, maintain, and submit timely clinical notes reflecting the patient’s condition and care provided.
  • Document changes in pain and symptom management, including outcomes and interventions.
  • Communicate proactively with physicians regarding patient needs and condition changes; obtain and implement physician orders as required.
  • Coordinate care by collaborating with community health professionals and resources.

Patient Care & Education

  • Provide medications, dietary guidance, and other instructions as prescribed by the physician.
  • Identify and utilize opportunities for comfort counseling with patients, families, and caregivers.
  • Deliver care in alignment with the interdisciplinary team to ensure holistic support.

Safety & Support

  • Maintain a safe, clean, and supportive environment for patients at all times.
  • Assist patients, families, and team members in delivering compassionate, comfort-focused care.
  • Support the emotional and psychosocial needs of patients and families in collaboration with the hospice team

Team Collaboration

  • Actively participate in interdisciplinary group (IDG) meetings to ensure coordinated, patient-centered care

Requirements

Qualifications

We’re looking for a compassionate and skilled RN Case Manager who meets the following criteria:

  • Graduate of an accredited school of nursing
  • Minimum 1 year of experience as a hospice case manager (preferred)
  • Current RN licensure in the state of practice
  • Strong observation, verbal and written communication, and problem-solving skills
  • Proficient in basic math and core nursing competencies
  • Comfortable with prolonged walking or standing
  • Able to lift, position, and transfer patients, as well as supplies and equipment
  • Physically capable of reaching, stooping, bending, kneeling, and crouching
  • Possess the visual acuity and hearing required to perform essential nursing tasks
  • Must be a licensed driver with a reliable, insured vehicle that meets state and organizational standards
